    This patient’s HbA1c had jumped to 12%, which is... < Back Tria Triumph 514 At Tria Health, our pharmacists review lifestyle habits to uncover any barriers to a patient’s care. #TriaTriumph A patient that is at a high risk for developing significant complications from diabetes called in for a consultation. This patient’s HbA1c had jumped to 12%, which is remarkably higher than their goal of less than 7%. Our pharmacist discovered that this patient was adherent with their diabetes medications. Furthermore, there was nothing overly concerning about this patient’s diet. However, when asked about beverage consumption, this patient shared that they drank at least one gallon of milk each day. While milk in moderation can be beneficial, a gallon of milk a day contains almost 2000 calories, 72 grams of fat, and an entire day’s worth of carbohydrates. The patient stated that no other health care professional had ever asked about their beverage consumption, so they were unaware of the problems this was creating for their diabetes and blood sugar control. Because of the #TriaHealth program, this patient agreed to make immediate changes to their fluid intake, mainly substituting water for most of their milk. Identifying the source of this patient’s calorie and carbohydrate consumption will have a dramatic impact on their diabetes control. Previous Next

    Prior to taking a blood thinner, this patient was on... < Back Tria Triumph 73 Tria Health provides a patient-centered approach, ensuring all aspects of a patient's health are properly managed. #TriaTriumph When patients get presented with a new diagnosis, providers can sometimes focus solely on prescribing treatments for the new problem without considering the patient’s other conditions and medications. A patient called into a consultation that had recently been diagnosed with atrial fibrillation. This patient started on a blood thinner that prevents strokes in the setting of atrial fibrillation. However, this medication comes with risk as any blood thinner can lead to bleeding complications. Prior to taking the blood thinner, this patient was on a medication for arthritis pain. This medication is very expensive, almost $30,000 per year. Furthermore, long-term use of this medication can lead to an increased risk of bleeding complications, specifically bleeding stomach ulcers. With the addition of the blood thinner, his arthritis medication became a potentially dangerous choice for the patient. Our pharmacist reviewed the risks with the patient and they were able to switch to a safer over-the-counter alternative for arthritis pain. In today’s #TriaTriumph , we see how the careful consideration of the patient’s condition was used to identify the correct dosage of the prescription that they needed. This patient had a history of atrial fibrillation and was taking Xarelto 15mg twice daily for stroke prevention. Xarelto is a blood thinner and a critical medication in patients with high stroke risk. However, when used inappropriately, it can cause a brain hemorrhage. Because of this, the right dose is crucial. Based on all factors, this patient should not have been taking more than 20mg daily. While a 10mg dosage difference seems like a small number, minor dosing changes are significant with this potent medication, and can lead to major complications. Tria’s pharmacist contacted the prescribing cardiologist, who agreed with her recommendations and adjusted the dose accordingly. A patient that is undergoing fertility treatments to achieve her dream of pregnancy called in for a consultation. Despite close follow up and monitoring during this treatment by her OB/GYN, PCP, and fertility specialist, she has continued to take Atorvastatin for her history of high cholesterol. Although this is a preferred and ideal treatment choice for high cholesterol, statin use is contraindicated in pregnancy as they are known to cause potentially serious birth defects. Women who are pregnant or trying to become pregnant should avoid all statin use. Our pharmacist discovered that this patient’s treatment team had never assessed or discussed her statin use. It is not uncommon in healthcare to focus on a specific issue and fail to review the big picture, and the impact of all aspects of treatment and their effects for patients.
