When patients get presented with a new diagnosis, providers can sometimes focus solely on prescribing treatments for the new problem without considering the patient’s other conditions and medications.

A patient called into a consultation that had recently been diagnosed with atrial fibrillation. This patient started on a blood thinner that prevents strokes in the setting of atrial fibrillation. However, this medication comes with risk as any blood thinner can lead to bleeding complications.

Prior to taking the blood thinner, this patient was on a medication for arthritis pain. This medication is very expensive, almost $30,000 per year. Furthermore, long-term use of this medication can lead to an increased risk of bleeding complications, specifically bleeding stomach ulcers. With the addition of the blood thinner, his arthritis medication became a potentially dangerous choice for the patient.

Our pharmacist reviewed the risks with the patient and they were able to switch to a safer over-the-counter alternative for arthritis pain. Until our intervention, none of their providers had identified this issue. The #TriaHealth program saves money and lives.
