About Tria Health

Tria Health is an innovative chronic condition management (CCM) company

  • Team of pharmacists who provide telephonic clinical services with patients including:

    • A to Z disease state review – from diabetes and hypertension to lung disease and specialty conditions; our average patient has 4-5 chronic health conditions

    • Critical evaluation and optimization of medication therapy – our average patient takes 10-11 medications

    • Collaboration with providers for continuity of care – our average patient sees 2-3 providers and over 90% of pharmacist recommendations are accepted

  • Additional services include remote patient monitoring, pharmacogenomics, tobacco cessation, and weight loss management

  • Our patient population:

    • Lives nationwide; urban and rural

    • Comes from a variety of backgrounds and cultures; translators are onsite for Spanish-speaking patients

    • Varies widely in age, professions, education, and health literacy levels

Albers Medical Pharmacy is a local, family-owned pharmacy since 1976:

  • Focused on personal connection and outstanding individualized patient care via one-on-one relationships with a pharmacist

Residency Activities Include:

  • Complete on average over 800 patient consults/year

  • Teaching certificate through the University of Missouri-Kansas City School of Pharmacy

  • Precept APPE students from multiple schools of pharmacy

  • Research project that includes manuscript preparation and podium presentation

  • Business plan development

  • Quality improvement project

  • Participate in organizational planning meetings and processes

Eligible Candidates

  • Anticipated graduation from an ACPE accredited school/college of pharmacy

  • Eligible for pharmacist licensure in Kansas and Missouri within 120 days of start of program

  • Complete a drug screen and background check prior to employment
